How to add structure and guidance to the response form?
When building the response form, the Header
and Description
can be utilized to guide the user better throughout the form. Creating a Header
or a Description
can be achieved by following these steps:
- In the Template Designer, navigate to the
Form
tab - Click
Add question
- Select
Header
orDescription
in theType
dropdown-
Header
is typically used to break groups of questions into larger sections that guide the user for input. -
Description
is typically used together with headers to provide additional guidance.

Text
This will be displayed to end-user-
Reference name
: Will be prepopulated based on the question title. The reference name will be displayed when utilizing the form responses smart field and is also theName
utilized in the binding syntax. Optionally, set additional settings to the question -
Spacing before
: Edit the space between the question and the elements above it in the response form (none, medium, or large). -
Space after
: Edit the space between the question and the elements below it in the response form (none, medium, or large).
